- Title from earlier librarians' description, which is based on description from catalog of the Rimbault library sale.
- Page numbers added by a later hand.
- Dating based on when Hawkins was engaged in research for his book "A general history of the science and practice of music," although he was a music researcher from about 1750. See the entry "Hawkins, John" in Oxford [Grove] Music Online.
- Original binding discarded when volume was rebound on September 17, 1936; in 1991 that binding was discarded and the volume was conserved, with each page being encapsulated.
