Summary

Health Outcome Measures in Primary and Out-Patient Care explores a wide range of scientific and care provider issues relating to health outcome assessment in primary and hospital out-patient care (ambulatory care). It takes a practical approach to the issue; many of the contributions are based on work currently in progress.
In parallel, more fundamental concepts concerning the principles of outcome measurement in chronic disease are addressed, and the limitations and constraints arising from the methods and contexts in which measures have been developed and applied are discussed.
Contributions have been drawn from a wide range of disciplines and from researchers and practitioners of outcome assessment across the United Kingdom and mainland Europe. This book is aimed at those who want to turn outcome assessment into a practical tool for routine clinical practice, decision making and policy formulation. It will be of interest to a broad readership of researchers, clinicians, policy makers and patients' representatives, from both hospital and primary care settings.
